How does Semaglutide work?

Ozempic vs Wegovy vs Rybelsus

Semaglutide and appetite

Reduced hunger, increased fullness, and lower energy intake are important contributors to the weight-loss effects of Semaglutide. People often describe this more casually as reduced appetite or less 'food noise,' but the medication acts through broader GLP-1 signaling involved in appetite, gastric emptying, glucose regulation, and metabolic physiology.

Semaglutide and type 2 diabetes

Semaglutide was originally developed in the diabetes-treatment context. FDA-approved Ozempic is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes. Kidney outcomes have also been studied in people with type 2 diabetes and chronic kidney disease. In the FLOW trial, Semaglutide reduced the risk of the primary composite kidney and cardiovascular outcome by 24% relative to placebo. FDA-approved Ozempic labeling now includes reduction of the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in adults with type 2 diabetes and chronic kidney disease.

Semaglutide and MASH

In August 2025, FDA approved Wegovy under the accelerated approval pathway for adults with noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) and moderate-to-advanced liver fibrosis. In the interim Phase 3 analysis supporting the approval, MASH resolution without worsening fibrosis occurred more often with Semaglutide than placebo, and improvement in liver fibrosis without worsening MASH was also more frequent. Confirmatory follow-up is ongoing to verify longer-term clinical benefit.

Semaglutide vs Tirzepatide

Semaglutide primarily activates GLP-1 receptors, while Tirzepatide activates both GIP and GLP-1 receptors. In the head-to-head SURMOUNT-5 trial in adults with obesity without type 2 diabetes, Tirzepatide produced greater average weight reduction than Semaglutide at 72 weeks. That trial result describes group averages and does not mean every individual will respond better to Tirzepatide.

Semaglutide vs Retatrutide

Semaglutide is primarily a GLP-1 receptor agonist. Retatrutide is an investigational triple receptor agonist that activates GLP-1, GIP, and glucagon receptors. Semaglutide is available in multiple FDA-approved prescription products, while Retatrutide remains investigational as of 2026.

Common Semaglutide side effects

The most common adverse effects of Semaglutide are gastrointestinal. Depending on the product and population, these commonly include nausea, vomiting, diarrhea, constipation, abdominal pain or discomfort, indigestion, and decreased appetite. Gastrointestinal symptoms are a major reason some participants discontinue treatment in clinical trials.

Important Semaglutide safety considerations

FDA-approved Semaglutide products include important warnings and precautions.
