ACTH(4-7)-Derived Heptapeptide
Semax is a synthetic heptapeptide derived from an ACTH fragment and is discussed for neuroprotection, cognition, and stroke recovery. Human studies exist, largely from Russian clinical literature, but the evidence base and U.S. regulatory context are very different from an FDA-approved neurological therapy.

Human intervention evidence
Human studies exist in ischemic-stroke and rehabilitation settings, but much of the literature is older, regional, and not equivalent to a modern U.S. approval program.
Human biology evidence
Clinical and mechanistic literature has explored neurological outcomes, BDNF, inflammatory signaling, and neurotrophic pathways.
Key takeaway
Semax has human neurological research, but the evidence should not be generalized into a validated healthy-person cognitive-enhancement therapy.
Evidence labels describe the research base and regulatory context; they are not treatment, dosing, or use recommendations. Review dates describe the last BACToBasics source check for this evidence summary. Anti-doping rules can change and should be verified with the applicable authority.

Semax Reconstitution Math
Reconstitution math starts with the amount contained in the vial and the liquid volume used in the calculation. Dividing the vial amount by the liquid volume gives the resulting concentration.
Example vial amount
10 mg
| Liquid Volume | Calculation | Concentration |
|---|---|---|
| 1 mL | 10 mg ÷ 1 mL | 10 mg/mL |
| 2 mL | 10 mg ÷ 2 mL | 5 mg/mL |
| 3 mL | 10 mg ÷ 3 mL | 3.3333 mg/mL |
This table demonstrates concentration math only. The liquid volumes shown are examples for comparison and are not preparation recommendations.

What is Semax?
Semax is a synthetic peptide derived from the ACTH(4-7) sequence with an added Pro-Gly-Pro segment. It was designed to retain neurobehavioral activity without the full hormonal activity of ACTH.
What human evidence exists?
Published clinical literature includes studies in ischemic stroke and cerebrovascular disease. Much of this literature is Russian-language, relatively small, and not equivalent to the large multicenter evidence typically expected for a modern U.S. approval.
Stroke and rehabilitation research
Older controlled clinical studies and later rehabilitation research reported neurological or functional improvements in patients receiving Semax alongside standard care. These findings are hypothesis-supporting, but trial design, regional practice patterns, and reproducibility matter when interpreting them.
Mechanism claims should stay separate from outcomes
Experimental work has explored BDNF, inflammatory signaling, and neurotrophic mechanisms. Changes in biomarkers or proposed mechanisms do not by themselves prove clinically meaningful cognitive or neuroprotective benefit.
Current U.S. compounding context
FDA reviewed Semax-related bulk drug substances in 2026 through the Pharmacy Compounding Advisory Committee process. FDA materials highlighted limited clinical-evidence quality and safety/characterization questions, so BACToBasics does not present Semax as an FDA-approved U.S. neurological drug.
Semax concentration math
BACToBasics can calculate concentration from user-entered vial mass and liquid volume. It does not recommend a Semax dose, route, preparation method, or cognitive/neurological treatment protocol.

Semax FAQ
Tap a question to expand only the answer you want to read.
Does Semax have human studies?
Yes. Human studies have been published, especially in ischemic-stroke and rehabilitation settings, though much of the literature is regional and older.
Is Semax FDA-approved in the United States?
BACToBasics does not represent Semax as an FDA-approved U.S. medication. FDA reviewed Semax-related bulk substances in the 2026 compounding process rather than as an approved neurological drug.
Does Semax improve cognition in healthy people?
The available clinical evidence does not establish a broadly validated cognitive-enhancement indication in healthy people.
Does BACToBasics recommend a Semax protocol?
No. The site provides evidence context and user-entered concentration math only.
