What is PEG-MGF?

PEG-MGF refers to a pegylated synthetic peptide associated with the mechano growth factor literature. It is commonly marketed as a modified, longer-lasting version of a synthetic MGF peptide. PEG-MGF is not an FDA-approved drug, and FDA states that it has not identified human exposure data for drug products containing PEG-MGF administered by any route.

What does MGF mean?

The term mechano growth factor, or MGF, originally arose from research on mechanically responsive IGF-1 gene expression in skeletal muscle. In humans, the IGF-1Ec transcript is often referred to as MGF because its expression can increase after mechanical loading or tissue injury. This endogenous transcript and its processing should not automatically be treated as equivalent to the synthetic peptide products sold online as MGF or PEG-MGF.

Endogenous MGF vs synthetic MGF peptide

A major source of confusion is that MGF can refer to an IGF-1 splice transcript as well as to a synthetic peptide derived from part of the IGF-1Ec E-domain. A detailed endocrine review noted that the synthetic 24-amino-acid peptide often called MGF had shown biological effects in experimental systems, but an analogous free peptide product had not been identified or isolated from cultured cells, conditioned media, animal tissues, or biological fluids. Claims about a synthetic MGF peptide therefore should not simply be presented as if they describe a known circulating human hormone.

What does pegylation mean?

Pegylation means attaching polyethylene glycol, or PEG, to a molecule. In drug development, pegylation is often used to alter properties such as degradation, solubility, distribution, or persistence in circulation. Online descriptions of PEG-MGF commonly state that pegylation makes the synthetic MGF peptide longer acting, but there is no established human pharmacokinetic study of commercially marketed PEG-MGF products that validates a specific duration of action.

What does laboratory research on MGF show?

Research on MGF-related biology is mixed and is largely preclinical. Some experiments using synthetic MGF E-domain peptides have reported effects on muscle progenitor cells, cell survival, or regeneration-related pathways. For example, one study found that an MGF E-peptide influenced human muscle progenitor cells in culture. These cell-culture findings do not establish that injected PEG-MGF improves muscle growth or recovery in people.

Not all MGF experiments have been positive

The MGF literature is not uniformly supportive. A 2014 study reported that the proposed MGF peptide had no apparent effect on the tested myoblast and primary muscle stem-cell systems, challenging some earlier claims about a distinct growth-promoting MGF peptide. This disagreement is another reason to avoid treating the mechanism or benefits of synthetic PEG-MGF as settled science.

Does PEG-MGF build muscle?

There is no reliable human clinical evidence showing that administered PEG-MGF increases muscle mass, strength, hypertrophy, or athletic performance. Muscle and cell studies involving endogenous IGF-1 splice variants or synthetic MGF-related peptides provide hypotheses, not proof of a bodybuilding effect in humans. Marketing claims about muscle gain should therefore be treated as unvalidated.

Does PEG-MGF improve recovery or injury healing?

MGF-related gene expression is associated with tissue responses to loading and injury, which is one reason PEG-MGF is marketed for recovery. However, there are no established human trials showing that administered PEG-MGF accelerates recovery from exercise, repairs muscle injuries, heals tendons or ligaments, or improves rehabilitation outcomes.

PEG-MGF vs IGF-1 LR3

PEG-MGF and IGF-1 LR3 are both discussed in performance-enhancing peptide communities, but they are different compounds. IGF-1 LR3 is a modified IGF-1 analogue designed to reduce binding to IGF-binding proteins, whereas PEG-MGF is marketed as a pegylated synthetic MGF-related peptide. Neither should be assumed to reproduce normal endogenous IGF-1 signaling in a physiologic way, and neither has an approved bodybuilding or recovery indication.

What does the 2026 clinical literature say?

A 2026 endocrinology review of performance-enhancing peptides included PEG-MGF among unregulated GH-IGF-1-axis compounds and highlighted the gap between online self-administration practices and clinical evidence. The review emphasized uncertainty in product composition, dosing, stacking practices, adverse effects, and efficacy, with some compounds in this market having no human studies at all.

FDA safety status

FDA currently lists PEG-MGF among bulk drug substances that may present significant safety risks in compounding. FDA states that compounded PEG-MGF may pose significant immunogenicity risk for certain routes and may involve complexities related to peptide impurities and API characterization. FDA also states that it has not identified human exposure data for PEG-MGF administered by any route and lacks important information needed to determine whether it could cause harm in humans.

Current U.S. compounding status

FDA's April 2026 update states that the nomination for Mechano Growth Factor, Pegylated was withdrawn, so PEG-MGF was removed from category 2 of the 503A bulk-substance nomination list. FDA also announced that it intends to consult the Pharmacy Compounding Advisory Committee regarding possible inclusion of PEG-MGF on the 503A Bulks List before the end of February 2027. This review process does not mean that PEG-MGF is FDA-approved.

What are the potential risks?

Because adequate human exposure data are lacking, a reliable adverse-effect profile for PEG-MGF has not been established. Potential concerns include immune reactions, peptide aggregation or impurities, product misidentification, uncertain potency, and risks related to manipulating growth-factor pathways. Risks inferred from IGF biology or other growth factors should not be presented as proven PEG-MGF adverse effects, but the absence of evidence should also not be mistaken for evidence of safety.

Is PEG-MGF prohibited in sport?

Yes. The 2026 World Anti-Doping Agency Prohibited List includes mechano growth factors under section S2.3, Growth Factors and Growth Factor Modulators. Substances in this category are prohibited at all times, both in and out of competition.

Research product vs regulated medication

A vial sold online as PEG-MGF is not equivalent to an FDA-approved pharmaceutical product. Research-market products may not have independently verified identity, sterility, concentration, purity, stability, or pharmacokinetics. A label stating a peptide name and quantity does not establish that the vial contains that exact compound or amount.
